Act 151: Criminal History Check

To complete the Criminal History Check, submit a New Record Check at the Pennsylvania Access to Criminal History(PATCH) website.The Pennsylvania State Police request for Criminal Record Check is to be completed by requester (information will be mailed to requester only).There is a $8.00 nonrefundable fee for each request, regardless of outcome. Payment is accepted by credit card or by billing a pre-established account.

Act 34: Child Abuse History Clearance

To complete the form, please visit the Pennsylvania Child Welfare Information Solution(PACWIS) website and create an individual account. There is a $8.00 fee to process documents payable via debit or credit card.

Act 114: FBI Fingerprint Clearance Check

Please follow the link to fill out the Pennsylvania Department of Education (PDE) FBI Federal Criminal History Check for Prospective Employees at Applicant Fingerprinting Online Services.Applicants may make their payment of $27.00 online using acredit card or debit card.
