Change to Graduation Application
This iform is for students who want to update the following information on their graduation application: Diploma name, diploma mailing address, graduation date, cancel their application, and/or ceremony attendance.
Graduate Course Repeat Request form (downloadable PDF)
Graduate students may repeat two “C” or “F” grades.
Request for Graduate Transfer Credit Review
This iform is for graduate students who would like to transfer credits from another accredited institution or from another program within IUP.
Request for Maximum Credit Load Increase
This iform is for graduate students who want to take more than the maximum allowed credits for one semester.
Request for Program Withdrawal - Graduate Students Only
This iform is for graduate students who want to withdraw from the current semester, program, or the university.
Undergraduate Application for Graduate Coursework
This iform is for undergraduate students who want to take a graduate course.
