Late Drop Add Form - may be accepted up to five days after add/drop ends with approval of instructor, chairperson, and assistant dean
Late Withdrawal Form - may be accepted up to five days after course withdrawal deadline with approval of instructor, chairperson, and assistant dean
Catalog Year Change Form - review determining UG catalog year information and speak to your academic advisor prior to filing this form
Incomplete Grade Form - review IUP Incomplete Policy
To change major, or declare or modify a double major or dual degree, minor, and certificate, use the iform on MyIUP under the Academics tab, Modify Academic Record box.
