All IUP students must complete a health history and immunization form on the Health Service web portal.
How to Submit

Only currently registered students will be able to submit information to their electronic medical records via this link.

IUP students must use their user name and password to enter the site.

Complete the Profile and Forms on the portal.

This page will also allow you to view and print the immunizations that you have on file with the Health Service whenever you choose.
